Liability Waiver

By registering for Global Sports Academy Futures Program, you agree, as a precondition to participate, you will not sue Global Athletic Academy LLC, and release GAA from any and all liabilities, claims, demands, actions, causes of actions, costs and expenses of any nature whatsoever arising out of any loss, damage, or injury, including death, that may be sustained by you or to any property belonging to you, or used by arising from the Activity or while upon the premises where the Activity is being conducted. You agree to "indemnify and hold harmless" GSA/GAA from and against any loss, liability, damage or costs, including court costs and attorneys’ fees, that the GSA/GAA may incur arising from your involvement in the Activity. "Warranty of Physical Fitness" You warrant that you or your child are physically fit and in a condition that will allow you/my child to participate fully in the Activity. You maintain medical insurance that covers you for accidents and illnesses while you are participating in this Activity. You understand GSA/GAA has not made, nor will make, any investigation into your physical fitness or ability to participate in the Activity, and the GSA/GAA is relying on your warranty of your/your child's physical condition. You assume full responsibility for payment of medical expenses not covered by your insurance incurred as a result of your participation in the Activity. "Emergency Medical Treatment". You grant the GSA/GAA and staff permission to authorize emergency medical treatment as it deems appropriate, and agree that such action by the GSA/GAA shall be subject to the terms of this Agreement. You understand and agree that the GSA/GAA assumes no responsibility for any injury or damage that might arise out of or in connection with such authorized emergency medical treatment. In signing this Agreement digital signature (typed out full name), you acknowledge that you have read this Release Agreement, understand it, and agree to be bound by its terms. You further acknowledge that you are signing this Release Agreement voluntarily and you are at least eighteen years of age.
